Our Vision
Carolina Kidney Partners will engage physicians, patients, and payers to improve the health of patients with chronic kidney disease. This collaborative community will radically transform the delivery of healthcare to patients with chronic kidney disease.
Operations
Carolina Kidney Partners provides a physician-driven, value-based system of care to patients struggling to manage their chronic kidney disease. With a growing network of nephrologists available throughout the region, patients can access their care wherever they need it.
Compassionate, on-staff care coordinators arrange and facilitate the many appointments, and providers required to manage chronic kidney disease (including nurses, pharmacists, social workers, and dietitians). Our unique synthesis of care coordination, education, analytics, contracting, and risk-sharing mitigates the complications of chronic kidney disease in a region with highly susceptible populations.
Offerings
With the Carolina Kidney Partners delivery system, a nephrologist-led team of coordinators and nurses manage the care of patients and work collaboratively to slow the progression of chronic kidney disease through innovative solutions to chronic problems.
Our coordinators render patient care more accessible by offering education on nutrition, arranging transportation, providing social work and behavioral health consultations, along with medication reconciliation. These coordinators are with patients at every step in the process, helping them navigate chronic kidney disease at each stage of their journey.
