Care Coordinator Job Description
The Care Coordinator is charged with an intense focus on improved patient outcomes and better patient engagement in the CKP value-based system.
Data Analytics
The CC will coordinate with the CMO, our analytic system, and nephrology providers to implement clinically meaningful improvement in patient outcomes. This data will then be used to measure quality outcomes, population costs, and patient satisfaction. The CC will provide feedback to the medical groups, providers, and company (CKP). The CC will contribute to action loops to implement change in individual patient care, population care, and cost evaluation.
Care Coordination
The CC will assist with and monitor the CKP care coordination system. CKP is focused on people to people interactions. The CC will develop methods to direct individual patients to the correct provider. This is a clinical role. We envision the CC will devote 85% of effort to CC of patients. We will embed the care coordinator in an individual nephrology practice to use the system that is developed. Our thought is that this will inform the further development of CKP.
In addition, the CC will function as an embedded CC - Additional roles:
Dialysis education with an emphasis on home dialysis
Transplant referrals and the data needed to make these efficient
Optimal dialysis start coordination for all patients expected to need a dialysis start within 6 months
Individual CC of patients in concert with their providers
Responsibility for CMMI metrics that impact quality
Supervision of the clinical care navigators
The care coordinator goals:
Contribute to development of dialysis education products
Contribute to the development of transplant education products
Responsibility for optimal dialysis starts metrics
Responsibility for CMMI metrics collection
Responsibility for the development of transitional care, and end of life counseling referrals
The care coordinator will contribute ideas to policies and procedures for the flow of information
Education/Discipline Requirements
Registered Nurse (RN), or
Masters in Social Work (MSW), or
Experience Care Coordinator, or
Renal Care Coordinator
